*Membership cannot be transferred to another office, or another person. Accounts must be active and in good standing to receive membership benefits. It is the patient's responsibility to schedule and keep appointments. There are no refunds of yearly fees for any reason, including failure to schedule or maintain appointments, relocation or lack of use. Some other restrictions may apply.
